Please note that we do not allow entries from academy and EJA / JPL teams.
Everything with an * is a required field
If you wish to enter multiple teams across different age groups, please submit a copy of this form for each age group.
Entry fee is £40 per team and £30 for each additional team in the same age group.
Your entry will only be validated on receipt of the tournament entry fee. This should be paid within 7 days of entering to:
